Your employer reports your hours worked and sends payment for those hours to OpenCircle Benefits. Once your Hour Bank account reaches 300 hours, your benefit coverage will begin on the first day of the second month that follows.

You have up to eight months to accumulate the required 300 hours. If you do not reach 300 hours within that time, all accumulated hours will be forfeited, and your account will reset to zero.

Each month of benefit coverage requires a deduction of 150 hours from your Hour Bank.

· Accidental Death & Dismemberment

     Principal Sum: $100,000

· Dependent Life Insurance

     Spousal Benefit: $10,000

     Child Benefit: $5,000

· Extended Health Care for Employees & Dependents: 

     Prescription Drugs: 80%

     All Other Covered Health Expenses: 100% 

     Vision Care Maximum: $350

     Prescription Safety Glasses: $150 

· Dental Care for Employees & Dependents 

     Basic Services: 80%

     Major Services: 50%

     Maximum for Basic & Major Services Combined: $2,500 

     Orthodontic Services: 50%

     Maximum for Orthodontic Services: $2,500 lifetime

· Long Term Disability Insurance

     Monthly Benefit: $2,500 per month for the first 24 months
